3 simple steps to a solution.

#1. No cellphones allowed. Put a sign on the door and refuse to seat anyone who doesn't turn off their cell in front of the server at the door.

#2. If 10 minutes after being seated, they are not ready to order, show them the door and tell them they are not welcome back.

#3. If more than 2 minutes has passed between the meals arriving at the table and the customer asking for a re-heat. Tell them it is as-is. No re-heats. If they do not like it, show them the door and tell them they are not welcome back.

Problem solved.
